how to tell girlfriend that I know she cheated

I found some very very convincing evidence that my girl had sex with someone else while we were broken up but still talking. I understand if she did have sex with someone else but this not knowing for sure is pissing me off REALLY bad. I really want to talk to her about it and put these thoughts to rest. How do I bring this up politely? I really need to get this off my chest.

Somethings she says didnt add up, and I found a used condom under her bed that definitely wasnt mine. Also her friend tells me too.
