How do I get my provisional license when I've had a surname change?

I am having trouble getting my provisional liscence as I had my name changed when I was younger my name from farren to smith on my passport it is the same as my name on my birth certificate but I dont want my provisional liscence in that name im not ment to be known by farren what can I do should I just send my passport and my birth certificate and just have my provisional as farren or can somone tell me a solution plzzz

I had my name changed legally but the soliciters were I had it done they have shut down so the documents have been destroyed so theres no way of getting the name change deeds how do I go about resolving this

Honestly, you’ll have to use the name that is on all your identification, whatever name that happens to be.
