a gauge can be an instrument for measuring something from psi to the amount of liquid in a tank… or it could be a type piercing where a hole (larger than a standard piercing) is made in the ear lob so that the hole may be filled artificially with an object.

It could be a measurement or a part of a car, like TwiddleMuffin said. Or it could be a piercing to the ear where you gradually increase the size of the piercing creating a bigger and bigger whole.
