I don’t think you could? I mean, you can make a strap on dildo because a pen!s is outside of the body. but a v@gina is on the inside, so where would the hole in the underwear go?

Erm… I could give a serious answer to this, adding links with pictures of toys that might be suitable for the purpose. But then Colleen will have to move the entire question to the “mature” section and you will no longer be able to view the answer, as your profile says you are 14.

But there are toys for men - available at adult shops - that are a miniature version of inflatable dolls for… solitary-use purposes.

You might be able to attach such a device to an undergarment if you can get one. You’d need to attach it so that it would lie between the legs of the person wearing the undergarment and would be… accessible from the front.
