psychologists dont use the term mad any more, but if you mean psychotic, well basically they’ve lost touch with reality… so they’re seeing or hearing things or feeling things that arent there (bugs crawling on their skin), or have beliefs that arent very likely (they’re jesus christ, they were abducted by aliens, people are talking to them through the tv, the government is out to get them) etc…
