Usually a yellow booklet where the record of shots you have received are recorded…Take care !!

I am a former principal and director of elementary education in Florida, and my experience is that In many states, your local health department will examine the record of immunizations (shots and vacinations) and give you a certificate of immunization that the schools will accept as meeting state law requirements for enrollment. The “certificate” is different from the “record”, as Amblessed implies. The record would probable come from the student’s doctor, or maybe the parent has kept a record of each required immunizations as they occurred. Either way, I doubt that the school will accept a shot record itself, unless they have a school nurse on staff, rather than the certification from some health authority that the student is up to date with requirements.
