You need a stethoscope, and you pump the blood pressure cuff up until you stop hearing a pulse on the down stream side of the cuff. Open the air release to start bleeding off the pressure of the cuff. When you first here a pulse in the stethoscope that number on the gauge will indicate the top number of the blood pressure. Continue to allow the pressure to decline until you stop hearing the pulse at the crook of the arm. This is the lower number. For example you start hearing the pulse at 120 and stop hearing it at 80 the blood pressure is 120/80. you can also count the pulse for vitals at the same time.
