Why my best friend might hate that I am not the same person?

To make a long story short, I was suppose to go with my best friend today to do something but in the end bailed since I had to take care of my aunt since she had a 100 temp, and top of that she is fighting cancer, and when I called her to say I can’t come she said it is ok, I am used to now. I will admit, I have said no on occasions since I was already doing something, but the majority of the time I have always said yes. I think this had all started since she started talking to an old friend since this behavior is recently. Is it wrong of me to say no since my aunt has cancer and was sick or does she need to realize my whole life does not revolve around her? To make things more simple, my aunt is the mother of my cousin who she hates right now based on some choices she has made. I have changed a lot in the years we have one another, and I am a lot less dependent on her as before, so I guess I just wanted to know do I have to feel like a flake for bailing or be okay with the choice I made?

She should learn to understand your situation right now. Find time to talk to her because things are better understood when they are explained.’
