How can I get my son to be responsible fpr himself

Problems with my boy 13yrs old . My son does not listen and cannot organise himself for school he is always forgetting things and I always have to be on his back, my husband tells me I do too much for him but if I don,t remind him of things he just don,t care if he has forgotton his school books or what ever. Can you suggest the best thing I have tried charts and routines.

hhmm…..my older bro is the same way tell him that if he duznt do whatever he will be grounded and if that duznt work tell him u will take stuff away from his room and stuff like that my mom did that and hes getting better :)
