When it says "patient print name and signature" does it mean like my name - Jen, and then "Jen" in cursive?

It just means you have to write your name then sign it with what ever signature you use, to show prove that it’s you

the reason for printing then signing is so that if your signature is illegible the form still bears your name and can be transferred to other forms if appropriate.

you should be printing your entire full name- first middle and last.

That means write your name as you would normally do so, and then your signature. If your signature IS in cursive than yes, otherwise it would just be whatever your signature is.

print means in non-cursive just normal and signature means in cursive or if your signature is print then just print it twice.
